Job summary

Montclare-Supportive-Living in Chicago, IL is seeking a CNA to deliver individualized care according to each resident’s care plan. You will assist with Activities of Daily Living while promoting independence and dignity, and communicate changes in health status to the clinical team.

Qualifications

• CNA certification required from an accredited program.
• Excellent communication and interpersonal skills.
• High School diploma or GED equivalent.

Responsibilities

• Provide individualized care per each resident's care plan.
• Assist with ADLs while promoting independence and dignity.
• Report changes in resident health status to the clinical team.
• Maintain confidentiality and privacy of resident information.
• Engage with residents and families with compassion and respect.
