Job summary

Montclare Supportive Living in Chicago, IL is seeking a dedicated CNA for part-time work on the 3rd shift to deliver compassionate, resident-centered care aligned with individual care plans.

You will assist with ADLs, monitor health changes, and communicate with residents and families to foster dignity and trust. The role includes ongoing education, confidentiality, and teamwork in a supportive environment.

Qualifications

• High School graduate or GED equivalent required.
• Current certification from an accredited CNA program.
• Exceptional communication and interpersonal skills.
• Must demonstrate the ability to serve with love, compassion, and dignity.

Responsibilities

• Deliver exceptional and individualized care as determined by each resident’s care plan.
• Provide assistance with Activities of Daily Living (ADLs) while promoting independence and maintaining dignity.
• Report changes in resident health status to the clinical team.
• Connect with residents and family members in a compassionate and impactful manner.
• Engage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences.
• Comply with all rules and regulations regarding confidentiality and privacy of resident information.
• Perform any other duties as assigned.
