Responsibilities

• Maintain accurate client and member records, documentation, and case files in HHA Exchange or Electronic Visit Verification.
• Communicate with caregivers, families, and healthcare professionals to support individualized care plans.
• Onboard new clients and members, guiding them through available services and agency processes.
• Monitor client satisfaction and follow up regularly to ensure quality of care.
• Support compliance with agency policies and home care regulations.
