Job summary

Vitality Hospice & Palliative Care seeks a compassionate Certified Home Health Aide (CHHA) to join our clinician-owned team. This full-time, field-based role supports patients at home under RN supervision, with a schedule of 7:00 AM–3:00 PM, Monday–Friday, and occasional evening/weekend overtime.

Qualifications

• Active Certified Home Health Aide (CHHA) certification is required.
• Valid driver’s license, reliable vehicle, and current auto insurance.
• Compassionate, reliable, and self-directed.
• Strong communication and time-management skills.
• Ability to work independently and follow care plans.
• Commitment to serving patients with chronic, serious, or terminal illnesses.

Responsibilities

• Assisting with mobility, transfers, and ambulation.
• Providing personal care (bathing, grooming, dressing, feeding).
• Offering companionship and emotional support.
• Performing light housekeeping tasks.
• Documenting care accurately and in a timely manner.
• Supporting a culture of compassion, quality, and respect.
• Working under the supervision of a Registered Nurse Case Manager.
