Job summary

Vitality Hospice is seeking a compassionate Certified Home Health Aide (CHHA) to join our field-based team in New Jersey. This full-time role is travel‑oriented, with mileage reimbursements and flexible scheduling, supporting patients at home with dignity and comfort.

You will work under RN supervision to provide essential personal care, mobility support, and companionship, contributing to a patient‑centered care plan and quality of life.

Qualifications

• Active CHHA certification required.
• Valid driver's license, reliable vehicle, and current auto insurance.

Certified Home Health Aide (CHHA) – Full-Time | Field-Based

Vitality Hospice & Palliative Care is seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to join our growing team. This is an opportunity to work for a clinician-owned and operated hospice that truly values both its patients and its employees.

Position Details

• Schedule: Full-Time, Monday–Friday, typically 7:00 AM–3:00 PM
• Occasional evening/weekend overtime available
• Field-based position with travel required (mileage reimbursed)
• Active CHHA certification required

What You'll Do

Working under the supervision of a Registered Nurse Case Manager, you will provide essential hands‑on care and support, including:

• Assisting with mobility, transfers, and ambulation
• Providing personal care (bathing, grooming, dressing, feeding)
• Offering companionship and emotional support
• Performing light housekeeping tasks
• Documenting care accurately and in a timely manner
• Supporting a culture of compassion, quality, and respect

You will be an integral member of our interdisciplinary team focused on clinical excellence and patient‑centered care.

Compensation

$18.00–$23.00 per hour , based on experience and qualifications
