Vitality Hospice & Palliative Care seeks a compassionate Certified Home Health Aide (CHHA) to join our clinician-owned team. This full-time, field-based role supports patients at home under RN supervision, with a schedule of 7:00 AM–3:00 PM, Monday–Friday, and occasional evening/weekend overtime.

• Active Certified Home Health Aide (CHHA) certification is required.

Certified Home Health Aide (CHHA) – Full-Time | Field-Based

Vitality Hospice & Palliative Care is seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to join our growing team. This is an opportunity to work for a clinician-owned and operated hospice that truly values both its patients and its employees.

Position Details

• Schedule: Full-Time, Monday–Friday, typically 7:00 AM–3:00 PM
• Occasional evening/weekend overtime available
• Field-based position with travel required (mileage reimbursed)
• Active CHHA certification required

What You’ll Do

Working under the supervision of a Registered Nurse Case Manager, you will provide essential hands‑on care and support, including:

• Assisting with mobility, transfers, and ambulation

Compensation

$18.00–$23.00 per hour, based on experience and qualifications

Work Location: In the Community – Patient Residences (Travel Required)
