Job summary

Vitality Hospice is seeking a compassionate Certified Home Health Aide (CHHA) to join our field-based team. You will provide hands-on care to patients in their homes under RN supervision, focusing on comfort, dignity, and quality of life.

The full-time schedule is typically Monday–Friday, 7:00 AM–3:00 PM, with occasional overtime. Mileage is reimbursed, and benefits include 401(k) matching, health/dental/vision insurance, PTO, bonuses, and paid training.

Qualifications

• Active CHHA certification required.
• Valid driver’s license, reliable vehicle, and auto insurance.
• Compassionate, reliable, and self-directed.
• Excellent communication and time-management skills.

Certified Home Health Aide (CHHA) – Full-Time | Field-Based

Vitality Hospice & Palliative Care is seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to join our growing team. This is an opportunity to work for a clinician-owned and operated hospice that truly values both its patients and its employees.

Position Details

• Schedule: Full-Time, Monday–Friday, typically 7:00 AM–3:00 PM
• Occasional evening/weekend overtime available
• Field-based position with travel required (mileage reimbursed)
• Active CHHA certification required

What You’ll Do

Working under the supervision of a Registered Nurse Case Manager, you will provide essential hands‑on care and support, including:

• Assisting with mobility, transfers, and ambulation
• Providing personal care (bathing, grooming, dressing, feeding)
• Offering companionship and emotional support
• Performing light housekeeping tasks
• Documenting care accurately and in a timely manner
• Supporting a culture of compassion, quality, and respect

You will be an integral member of our interdisciplinary team focused on clinical excellence and patient-centered care.

Compensation

$18.00–$23.00 per hour , based on experience and qualifications
