Grace Healthcare Services in New Jersey is hiring a Certified Home Health Aide (CHHA) for Middlesex, Somerset, and Union Counties. You will provide compassionate personal care, assist with daily activities, and support patients in their homes, delivering dignity and comfort at every step of the end-of-life journey.

Requirements include an active NJ CHHA license in good standing, the ability to read, write, and speak English fluently, and the capacity to lift up to 50 lbs.

Qualifications

• Must hold a NJ CHHA license in good standing.
• Ability to read, write, and fluently communicate in English.

Responsibilities

• Provide personal care: bathing, grooming, dressing, skincare, and mobility support.
• Meal preparation tailored to dietary needs.
• Maintain clean, safe, and comfortable patient living spaces.
• Document care activities using Celltrak and ensure clear communication.
• Collaborate with RN Case Managers to monitor patient changes.
