CT Health Care Services is hiring a Certified Home Health Aide (CHHA) in New Jersey. You will assist clients with ADLs, mobility, and daily routines under RN supervision, delivering compassionate, patient-centered home care.

You will document activities in EHR/EVV, observe changes in condition, and collaborate with the care team to ensure quality outcomes. Strong communication and dementia-care experience ideal.

Qualifications

• Active New Jersey Certified Home Health Aide (CHHA) certification in good standing.
• Strong verbal and written communication with empathetic, patient-centered approach.
• Experience assisting with ADLs and safe client transfers.

Responsibilities

• Assist clients with core Activities of Daily Living (ADLs) and mobility/transfer support using proper techniques.
• Maintain clear, compassionate communication with clients and family; report changes to RN.
• Provide dementia and memory-support care with empathetic approaches.
• Perform light household tasks tailored to client needs to maintain a safe home environment.
• Use EHR/EVV systems to document care activities and daily notes accurately.
