Qualifications

• Must have a State Certified CHHA license.
• Experience in hospice or home health preferred, but no experience is necessary.
• Must be willing to travel and have a valid driver's license.

Responsibilities

• Provide assistance with ADL activities including personal hygiene and grooming.
• Assist in toileting and transfer activities for patients.
• Report any changes in patient's condition to the Hospice Nurse.
