Job summary

Symbii Home Health in Wyoming seeks a Certified Nursing Assistant to provide compassionate personal care in patients' homes, including baths, grooming, feeding support, and safe transfer assistance in Farson-Eden, WY. You will follow the plan of care, monitor patient status, document care accurately, and cooperate with nurses and other staff.

The role requires 18+ years, state training, CPR, and a valid driver’s license; training is provided as needed.

Qualifications

• Meets training requirements per State and Federal laws.
• Must be at least 18 years of age.
• Ability to read and follow written instructions and document care.
• Self-directed with minimal supervision.
• Empathy for the ill, injured, frail and impaired.
• CPR certification required.
• Licensed driver with insured vehicle.

Responsibilities

• Provide personal care (baths, grooming, oral hygiene, etc.).
• Assist with dressing, toileting, and linen changes.
• Plan and prepare nutritious meals, assist feeding if needed.
• Take and record vital signs as ordered (temp, pulse, respirations, BP).
• Assist in ambulation and prescribed exercises.
• Follow plan of care and report changes to nurse.
• Perform basic procedures as trained and documented.
• Assist patient with medication self-administration.
• Maintain clean and safe living environment.
• Ensure safety and use equipment correctly (foot stools, rails).
