Qualifications

• Bachelor's Degree in Psychology, Social Work, or related field.
• Two or more years of Case Management experience.
• Ability to provide transportation to clients using VOA fleet vehicles.

Responsibilities

• Provide support to assigned clients and link to community resources.
• Coordinate with treatment team and clinicians.
• Maintain documentation of client progress and treatment plans.
• Advocate for clients to ensure access to resources and rights are respected.
• Conduct intake elements and admissions processes.
