Qualifications

• Bachelor’s degree in Social Work, Psychology, or a related field required.
• Previous case management experience preferred.
• Valid Government Issued Driver’s License required.
• Familiarity with coordinating and managing patient care plans, ensuring integrated services.
• Experience in crisis intervention for medical, social, or emotional challenges; HIV experience preferred.

Responsibilities

• Works scheduled hours within the clinic setting as part of the internal care team, serving all Vivent patients.
• Conduct thorough assessments of clients' medical and psychosocial needs to develop individualized case management plans.
• Collaborate with healthcare providers to coordinate access to medical services and holistic patient care.
• Perform eligibility assessments and collect eligibility proof documentation for grant funded services.
• Assist clients in accessing community resources, social services, and insurance coverage.
• Advocate for clients within the healthcare system to ensure needs are met.
• Provide crisis intervention services as needed.
• Maintain accurate documentation of case management activities in line with policies.
• Collaborate with interdisciplinary healthcare teams to achieve optimal health outcomes.
• Offer health education and support to empower clients in managing their care.
• Support other service lines when necessary (e.g., food pantry, prevention, telehealth).
• Adhere to agency policies including Confidentiality and Standards of Conduct.
