Qualifications

• Bachelor’s degree in social work, psychology, or human services-related field.
• Experience in behavioral health or case management is preferred.
• Strong communication and coordination skills are essential.

Responsibilities

• Accepts newly assigned cases after intake by a licensed provider and completes case management assessment.
• Develops treatment/service plans with patients to implement services and build alliances.
• Provides ongoing support and reviews to ensure goals are achieved and plans are updated as needed.
• Documents contacts timely and maintains case records per regulatory standards.
• Assists patients in obtaining appointments and referrals to community resources.
