Job summary

COMMUNITY CARE OF WEST VIRGINIA INC is seeking a dedicated Case Manager in Weston, WV to manage a caseload of adolescents and adults with mental illness and/or chemical dependency. You will assess needs, develop and implement treatment plans, and coordinate services for individuals and families served.

The role requires collaboration with mental health professionals, documentation in EHR, and regular reviews to ensure progress toward goals.

Qualifications

• Must possess a combination of behavioral health work experience or relevant education and training.
• Bachelor’s degree in social work, psychology, or human services‑related field is required.
• Two years of experience in a healthcare setting, preferably behavioral health.
• Experience with Medicare and Medicaid population, insurance providers, and telephonic engagement.
• Experience serving underserved populations.

Responsibilities

• Accepts newly assigned cases and completes case management assessment for patients.
• Develops treatment/service plans and coordinates services with patients and families.
• Provides ongoing support and case management per treatment plan goals.
• Conducts face‑to‑face reviews and adjusts plans as needed.
• Documents contacts and maintains case records per regulatory standards.
• Assists patients with external referrals and community resources.
• Maintains referral tracking and communicates with staff about progress.
