Job summary

Qualifications

• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.
• Valid driver’s license in good standing.

Responsibilities

• Deliver support per individual service plans, including socialization and health maintenance.
• Assist with daily activities: personal care, housekeeping, transportation, and community integration.
• Record progress, review logs, and maintain confidentiality.
• Ensure safety and follow policies during all activities.
