Job summary

Symphony Care Network, LLC. in Michigan is seeking a Caregiver to provide compassionate, person-centered care to Residents in Assisted Living. You will support daily living activities, monitor health changes, and maintain safety and confidentiality under HIPAA.

The role requires high school diploma or equivalent, CPR/First Aid certification, and ability to work nights and weekends. You will collaborate with an interdisciplinary team and document care accurately.

Qualifications

• High school diploma or equivalent.
• Knowledge of Assisted Living regulations.
• Current CPR/First Aid certification or ability to obtain certification within a specified timeframe.
• Completion of required facility and regulatory training including resident rights, infection prevention, abuse/neglect reporting, emergency procedures, and person-centered care.

Responsibilities

• Provide compassionate, respectful, and person-centered care.
• Observe and report changes in a resident's physical, mental, or emotional condition.
• Assist with activities of daily living including bathing, hair care, mouth care, nail care and dressing.
• Assists residents with ambulation.
• Assist residents with turning/repositioning, toileting and personal hygiene per care plan.
• Maintain confidentiality of resident health information in accordance with HIPAA.
• Participate in data collection and performance improvement reporting processes.
