Job summary

Sky Heart Home Care Services in Sanford, NC is hiring a compassionate Care Transport Associate (Driver) to safely transport clients to medical appointments and community destinations. You will assist with mobility, maintain vehicle safety, and coordinate with care teams to meet client needs.

Requirements include a valid driver’s license, reliable transportation, background check, and required CPR/First Aid and Defensive Driving training.

Qualifications

• Valid driver's license with no major violations in past 3 years.
• Must have own reliable transportation.
• Background check and drug screening required.
• CPR/First Aid, PASS training and Defensive Driving training required (provided by company).
• Min. availability of 5 days per week.

Responsibilities

• Safely transport clients to and from medical appointments, errands, and other destinations.
• Assist clients with boarding and exiting the vehicle, including those with mobility challenges.
• Maintain a clean, safe, and well-functioning vehicle at all times.
• Communicate effectively with care coordinators regarding scheduling and client needs.
• Monitor and report changes in client condition or behavior to staff.
• Adhere to traffic laws, safety regulations, and company driving policies.
• Provide courteous, patient-centered service to clients and families.
