Job summary

Wellstar Health System is seeking a Care Partner to support the patient care team under the direction of the Registered Nurse. The role focuses on promoting a positive patient experience by assisting with daily living activities and basic patient care, including ADLs, ambulation, and vital signs.

Exemplary communication, compassion, and a service mindset are essential as you collaborate with nurses, physicians and the healthcare team to deliver safe, age-appropriate and culturally aware care.

Qualifications

• High School Diploma or GED required.
• BLS (Basic Life Support) or ARC-BLS required on hire.
• Graduation from state-approved CNA or Patient Care Technician program required.

Responsibilities

• Excellence in patient experience by partnering with the healthcare team, patient and family.
• Provide patient care under the direction of the RN, including hygiene, nutrition, elimination and comfort measures.
• Monitor data entry for key patient safety information and participate in charge reconciliation.
• Assist in direct patient admission and support care delivery as assigned.
• Participate in hourly patient rounding and communicate promptly with RN/Charge Nurse.
• Maintain patient care documentation in the EMR and use safe body mechanics and equipment.
