NextStep Care is seeking a dedicated Case Manager to coordinate member care in our nonprofit settings. You will liaise with providers, educate families, and monitor wellbeing through visits and calls, ensuring high-quality outcomes.

Qualifications

• Bachelor's degree in social work or related human services field required with 2 years of work experience.
• Experience in social work, home and community-based services, healthcare or geriatrics preferred.

Responsibilities

• Acts as liaison between members and home and community-based service providers to facilitate quality and cost-effective care and outcomes.
• Provides education and support to members and their families regarding community resources and services.
• Develops collaborative relationships with primary care providers, managed care plans and HCBS providers.
• Maintains regular communication with members via phone and home visits to monitor wellbeing and adjust care plans.
• Records and documents interventions and member information accurately per guidelines.
• Coordinates with Central Intake and Assessment Team to ensure continuity of care.
