Job summary

BayMark Health Services, a BAART Programs company, seeks a compassionate Substance Use Counselor in the San Francisco area. You will conduct individual and group counseling, coordinate services, and help patients through medically assisted treatments to improve outcomes.

Qualified candidates are California-certified counselors with experience with chemically dependent individuals and the ability to complete assessments, treatment plans, and aftercare arrangements.

Qualifications

• Certified Counselor in the State of California.
• Previous experience working with chemically dependent individuals preferred.
• Satisfactory drug screen and criminal background check.

Responsibilities

• Provide individual and group counseling; direct service hours at regularly scheduled intervals; coordinate all services throughout treatment
• Completes Bio-psychosocial assessment.
• Facilitates, establishes and reviews with each patient the initial treatment plan
• Develop aftercare plans and discharge plans
• Ensure efficient and effective delivery of counseling services to all patients
• Other related duties as determined by supervisor
