Job summary

Circle of Life Hospice seeks a licensed social worker to support Bereavement Services, providing counseling to individuals and families facing grief, illness, or loss. The role emphasizes confidentiality, compassionate care, and collaboration with healthcare teams.

Minimum one year of related clinical experience in hospice or mental health settings is required; LMSW or LCSW certification is required in Arkansas. Daily use of EMR and participation in program development are expected.

Qualifications

• Minimum of one year related clinical experience in hospice, medical or mental health setting.
• Demonstrated knowledge of grief modalities, therapeutic interventions, and best-practices.
• Ability to maintain confidentiality and exercise good judgment with sensitive information.

Responsibilities

• Provide one-to-one and/or group counseling to those grieving the death of a loved one or living with a life-threatening or terminal illness.
• Prepare and maintain electronic clinical case files utilizing designated client tracking software; record client data and submit weekly clinical session logs/reports.
• Participate in staff, team, and relevant community meetings; contribute knowledge and experience to nurture a learning environment.
• Provide on-site and follow-up clinical interventions/consultations for hospice patients and families, including children and teens.
• Provide counseling to persons in crisis and critical incident, stress management and bereavement services.
• Maintain communication with primary care physicians, clinicians, and/or other individuals significant in each care plan.
• Conduct initial assessments, maintains and updates treatment plans as needed.
• Document contacts made with outside agencies and professionals as well as making referrals to community resources.
• Provide bereavement support for community members, debriefing and participate in community presentations.
• Consult clinical supervision on an as needed basis; Provide supervision for MSW interns.
• Utilize the Electronic Medical Records system on a daily basis.
• Participate in program development of the department.
• Provide back up to the social work team.
• Support Circle of Life staff and community professionals with grief and loss issues as needed.
• Follow bereavement policies and procedures.
• Accountable for direct delivery of bereavement care services provided by Circle of Life Hospice.
• Assess, plan, implement and document appropriate interventions.
• Demonstrate compassion, empathy, and patience when interacting with patients, families, co-workers, and members of the public.
• Represent Circle of Life to the community in a positive manner.
• Demonstrate flexibility, versatility and a positive attitude in integrating additional duties.
• Perform other duties as assigned.
