Job summary

Paycom is seeking a full-time Care Manager to provide ongoing supportive and case management services to individuals with long-term mental illness. Under supervision, you will participate in interdisciplinary meetings to align treatment planning, engage clients in services, and coordinate transitions between providers.

The role requires a 4-year degree and a valid Georgia driver’s license, with strong documentation and compliance responsibilities across care plans and notes within the care

Qualifications

• Bachelor’s degree from an accredited program in a relevant field (Psychology, Social Services, Social Work, etc.)
• Must possess a valid GA Driver’s license.

Responsibilities

• Provides on-going supportive and/or case management functions to individuals having long-term mental illness to help clients achieve goals.
• Participates in regular interdisciplinary staff meetings with the interdisciplinary team to support consumers.
• Gathers information for assessment, treatment planning, and connects individuals with service options.
