Job summary

Gateway CSB PEO LLC is seeking a case manager to provide ongoing support and clinical documentation for consumers with long-term mental illness. The role involves coordinating with an interdisciplinary team, meeting direct service hours, and engaging community resources to support clients’ goals.

Applicants should hold a Bachelor’s degree and at least 2 years of related experience, with a valid Georgia driver’s license and willingness to travel between Gateway sites.

Qualifications

• Bachelor’s degree from an accredited program and a minimum of 2 years related experience.
• Knowledge of crisis intervention techniques and treatment planning.
• Valid GA driver’s license and ability to travel between Gateway sites.

Responsibilities

• Document and maintain clinical records accurately within required timeframes.
• Ensure compliance with agency policies and standards in all notes.
• Collaborate with hospitals, shelters and community providers to engage and transition consumers.
• Meet minimum direct billed hours per year and deliver at least 4 face-to-face visits monthly.
