Job summary

Theoria Medical is seeking Advanced Practice Providers to join our post-acute network, delivering high-quality, patient-centered care in skilled nursing facilities nationwide. You will care for residents over time, building trust and continuity in a mission-driven team.

Daily rounds, medication/treatment planning, and on-site collaboration with facility staff are core. Our technology enables efficient charting, secure communication, and compliance with standards.

Qualifications

• Active U.S. license and board certification in an APRN or PA credential.
• Board-certified by ANCC, AANPCB, PNCB, or AACN for NPs, or NCCPA for PAs.
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Geriatric Medicine.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
• Communicate with facility nursing staff via phone or secure messaging as needed
• Maintain reliable on-site attendance for scheduled shifts
• Ensure compliance with clinical standards, facility policies, and regulatory requirements

What You’ll Do

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
