Job summary

Theoria Medical is seeking a full-time Advanced Practice Provider to serve local skilled nursing facilities within a close-knit, mission-driven care community. You will build trust through long-term patient relationships and provide high-quality, patient-centered care.

Qualifications

• Active U.S. medical license and board certification in the appropriate NP/PA boards (ANCC/AANPCB/PNCB/AACN or NCCPA).
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Perform daily rounding, prescribe medications and treatment plans, address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

• The estimated compensation range for this position is $42,000 to $118,000 per year, based on expected earnings under our uncapped productivity-based compensation model

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
