Meridian Senior Living seeks a Home Health Aide to provide direct care to residents during 12-hour shifts in Woodmere, NY. You will assist with daily activities, support residents' well-being, and work within the care plan while adhering to policies.

Responsibilities

• Provide direct care to residents per the customized service plan.

• Maintain cleanliness and order in residents’ living spaces.
