Job summary

Monarch Communities in Melville, NY, is seeking a Home Health Aide to support residents under the supervision of the Health & Wellness Director. The role involves assisting with personal care, daily living activities, and documenting services per the resident care plan.

Responsibilities include bathing, dressing, mobility, meal support, and ensuring residents’ safety and dignity on shift. Prior CHHA certification and experience with the elderly (dementia experience preferred) are valued assets.

Qualifications

• Certified Home Health Aide
• Knowledge and experience in working with the elderly population, preferred dementia experience

Responsibilities

• Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan
• Maintain the safety, comfort, dignity, and confidentiality of all residents
• Escort residents to meals and activities
• Ensures all assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts
• Document all services provided to your assigned residents during your shift
• Report any change of condition in a resident immediately to your supervisor
