Job summary

Prime Home Health and Companion Care in West Springfield, MA seeks a Homemaker/Companion to provide non-medical support in clients' homes, helping maintain a safe, clean, and comfortable environment. You will assist with household tasks, meals, and social engagement under supervision.

This role emphasizes client independence, safety, and respectful communication. Flexible hours across weekdays, weekends, evenings, and holidays, with travel within the service area.

Qualifications

• High School Diploma (preferred).
• Experience in home care or elder care preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and follow instructions.
• Reliable transportation and willingness to travel within service area.

Responsibilities

• Provide companionship, socialization, and emotional support to clients.
• Assist with meal planning and preparation according to dietary guidelines.
• Perform light housekeeping duties such as laundry, dishwashing, vacuuming, dusting, and general tidying.
• Assist with errands, grocery shopping, and escort to appointments or social activities (as applicable).
• Provide reminders for medications (without administering), appointments, and hydration.
• Observe and report changes in the client’s condition or home environment to the Nursing Supervisor.
• Communicate schedule changes, availability, and client concerns to the Scheduling Coordinator.
• Ensure the living environment is safe and free from hazards.
• Follow agency policies, procedures, and state regulations; maintain confidentiality; participate in training; clock in/out via EVV/GPS system.
