Telligen is seeking a Transition Support Coordinator to deliver face-to-face or virtual Specialized Transition Services to members transitioning to community living. You will collaborate with Care Managers, coordinate housing, navigate resources, and update care plans accordingly.

Requirements include a four-year health/related degree and a valid driver’s license with extensive local travel. Bilingual EN/ES is a plus; position in Cook/Lake counties and central IL (Decatur/Peoria) with Telligen’s

Qualificações

• Four-year degree in a health care or related field or relevant experience.
• Valid driver's license required and up to 95% local travel.
• Bilingual English and Spanish a plus.

Responsabilidades

• Provide face-to-face and/or virtual specialized transition services for persons with disabilities, behavioral health needs, and complex medical care needs.
• Collaborate with care management team, health care providers, and ancillary service providers to implement the care plan support.
• Document transition activities and update care plans as needed.
