Job summary

Thresholds is seeking a Transition Coordinator for the CCMTP within the Illinois Williams and Colbert Consent Decrees in Chicago. The role reports to the CCMTP Transition Team Leader and focuses on guiding Class Members from institutions to community living with Thresholds support services.

Qualifications

• Bachelor’s degree is required to meet rule 132 requirements for a Mental Health Professional.
• Experience working independently and in a team-oriented, collaborative environment.
• Proficient in e-mail, data reporting, record keeping and MS Office applications.
• Must have a valid driver’s license and daily access to a personal vehicle.
• Must obtain and maintain liability insurance coverage ($100,000/$300,000/$100,000) and meet DMV requirements.

Responsibilities

• Assess Class Member’s personal, medical, social, emotional, and environmental situation to plan for transition.
• Provide community support, engagement activities, and transition linkage.
• Provide brief interventions using evidence-based psychosocial treatments (CBT, MI).
• Support Class Members in developing independent living skills for community living.
• Assist housing location, community integration, and transition preparedness.
• Advocate for housing in the community and assist with documents and subsidy paperwork.
• Help Class Members move and set up their new housing.
• Prepare Class Members’ new housing by shopping for furniture and items.
• Transport members to appointments for discharge prep.
• Collaborate with CCMTP team on planning, safety, and problem solving.
