Job summary

Thresholds is seeking a Transition Coordinator, CCMTP in Chicago to join the Transition Team under the Comprehensive Class Member Transition Program. The role supports safe, community-based transitions for Class Members from institutional settings with Thresholds services in place.

Responsibilities include assessment, linkage to housing and services, and collaboration with the CCMTP interdisciplinary team. The position requires a Bachelor’s degree and a valid driver’s license, with opportunities

Qualifications

• Bachelor’s degree required to meet rule 132 requirements for a Mental Health Professional (MHP).
• Experience working independently and in a team environment.
• Proficiency with email, data reporting, record keeping and MS Office applications.
• Must be able to obtain and maintain required liability insurance and meet age requirements.

Responsibilities

• Assess Class Member’s personal, medical, social, emotional, and environmental situation to plan for transition.
• Provide community support, linkage and case management to support transition needs.
• Deliver brief interventions using evidence-based psychosocial treatments (CBT, MI).
• Assist in housing location, community integration and transition planning.
• Support Class Members with securing housing and completing subsidy paperwork.
