Job summary

Alliance Health seeks an In Reach and Engagement Specialist to support the Transition to Community Living (TCL) program. You will assist members with accessing services, coordinate with care teams, and build partnerships to identify high-risk individuals in Wayne County.

The role requires two years of behavioral health experience, strong MI and interpersonal skills, and ability to travel for community-based support. Hybrid work with onsite reporting in Smithfield, NC as needed.

Qualifications

• High school diploma or GED and 2+ years experience in behavioral health or related fields.
• Two years lived experience navigating Mental Health/Public Health/Social Service/Justice systems preferred.
• Bachelor’s or Master’s degree in human services related field preferred; IPS-SE and related trainings beneficial.

Responsibilities

• Complete assessments of individuals’ interest in community inclusion and housing options.
• Educate members about Medicaid, 1915(b/c) waivers, and community resources.
• Provide face-to-face and field support, including housing applications and community visits.
