Job summary

Telligen is seeking a Transition Support Coordinator to deliver Specialized Transition Services to members seeking community-based living options. The role supports care managers, documents transition outcomes, and educates members while traveling locally. The position involves close collaboration with health care providers and other team members to ensure appropriate care and seamless transitions.

Qualifications

• Requires a four-year degree in a health care or related field or relevant experience.
• Must possess a valid driver's license and willingness to travel up to 95% locally.
• Experience in case management or care coordination is a plus.

Responsibilities

• Provide face-to-face and/or virtual Specialized Transition Services.
• Collaborate with the care management team and health care providers to implement care plans.
• Document transition activities and update the care plan as needed.
• Deliver ongoing education to members and families.
• Communicate with health care providers to ensure appropriate care.
• Perform miscellaneous duties as assigned.
