Job summary

OCES is expanding to support older adults and individuals with disabilities by coordinating vital information and services. We are seeking a Case Assistant to support the Community Transition Liaison Program (CTLP) with administrative tasks, documentation gathering, and housing applications.

The role involves in-person and hybrid work, data entry, and interaction with healthcare and housing partners. Strong communication and MS Office proficiency are essential.

Qualifications

  • High School diploma or equivalent required.
  • Associate degree preferred.
  • Minimum one year of relevant experience.

Responsibilities

  • Supports the Community Transition Liaison to operate at the top of their role and responsibility
  • Accompanies the Liaison on nursing facility visits as needed
  • Provides additional support to the Liaison for complex cases
  • Gathers documentation needed to assist the consumer in applying for public benefits
  • Assists the consumer in completing and submitting housing applications
  • Conducts clerical duties including data entry into EOEA-designated cloud based data enterprise system

We are expanding to 11 more towns covering the south shore area, including Quincy, Braintree, Weymouth, etc. OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. By promoting healthy, safe living for our consumers, we hope to allow them to stay active and engaged in their lives and in their own communities for as long as possible. Our mission: Through the talents of an experienced and diverse workforce, OCES supports the independence and dignity of older adults and individuals with disabilities by providing essential information and services that promote healthy, safe living which positively impacts our community. We are seeking a Case Assistant to provide administrative support for our Community Transition Liaison Program (CTLP).

Job Duties/Responsibilities
  • Supports the Community Transition Liaison to operate at the top of their role and responsibility
  • Accompanies the Liaison on nursing facility visits as needed
  • Provides additional support to the Liaison for complex cases
  • Gathers documentation needed to assist the consumer in applying for public benefits
  • Assists the consumer in completing and submitting housing applications
  • Conducts clerical duties as needed by Liaison, including data entry into EOEA-designated cloud based data enterprise system
Required Knowledge/Skills/Abilities
  • Excellent verbal and written communication skills.
  • Excellent computer skills
  • Ability to respect the workplace and providers privacy and confidentiality
  • Ability to deal effectively with statistical and financial data
  • Knowledge of general office procedures and equipment
  • Proficient with Microsoft Office Suite or related software
Education/Experience
  • High School diploma or equivalent
  • Associate degree preferred
  • Minimum one year of relevant experience.
Work Environment
  • May need to work on-site at times as needed
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift-up to 25 pounds at times
  • Ability to travel and reliable means of transportation.
  • Must have personal computer with secured internet access at home
