Job summary

Qualifications

• Bachelor's degree in Human Services or related field with 2 years post-bachelor experience in Mental Health/substance use population.
• Master’s degree in a Human Services field with 1 year post-graduate MH/DD/SAS experience.
• RN licensure in North Carolina with 4 years MH/DD/SAS experience.
• Strong knowledge of evidence-based practices in mental health and substance use treatment.
• Excellent communication and interpersonal skills; ability to build rapport with diverse audiences.

Responsibilities

• Conduct comprehensive assessments to identify client strengths, needs, and goals.
• Develop, implement, and monitor tailored treatment plans with clients, families, and CST team members.
• Participate in PCP development and revisions, ensuring implementation.
• Consult with medical and non-medical providers; engage community supports; include input in PCP.
• Provide direct therapeutic interventions (individual, family, group) and crisis intervention.
• Coordinate and link clients to community resources; assist with vocational and housing needs.
• Maintain detailed documentation in EHR within 24 hours; ensure privacy and rights of clients.
• Assist Team Lead with regulatory documentation requirements and timelines.
