Job summary

Rockwell Development Center seeks a CST Qualified Professional Facilitator to develop, implement, and monitor tailored treatment plans in collaboration with clients and CST team members. You will provide individual, family, and group therapy, including crisis intervention, and coordinate linkage to community resources.

Responsibilities include initial assessment, PCP involvement, advocacy across systems, and maintaining compliant documentation within required timelines.

Qualifications

• Bachelor's degree in a Human Service or related field with 2 years post-bachelors experience with Mental Health or substance use population; meets requirements under 10A NCAC 27G.0104 as a Qualified Professional for Mental Health.
• Non-human service bachelor’s degree may be considered if candidate has 4 years of experience with mental health population.
• RN license in NC with 4 years MH/DD/SAS experience.
• Master’s degree in Human Services with 1 year full-time MH/DD/SAS experience.

Responsibilities

• Conducts comprehensive assessments to identify client strengths, needs, and goals.
• Develops, implements, and monitors tailored treatment plans with clients, families, and CST team members.
• Participates in development and revision of PCP and ensures implementation.
• Consults with medical and non-medical providers and includes input in PCP process.
• Provides crisis intervention and direct therapeutic interventions (individual, family, group).
• Links clients to community resources and assists with vocational, housing, and basic needs.
