Community Based/Day Program Therapist role includes diagnostic assessments, patient visits, and up-to-date treatment

Qualifications

• Degree in social work or related field with clinical therapy experience.
• Licensure as LSW/LISW/LISW-S/LPC/LPCC/LPCC-S/LMFT required or equivalent.

Responsibilities

• Performing comprehensive diagnostic assessments.
• Providing patient visits as scheduled.
• Ensuring treatment plans are up to date for each client.
• Completing comprehensive progress notes.
• Communicating and following up regarding client status.
• Working closely with case management and psychiatric services.
