Job summary

LifeBridge Health is seeking a patient navigation professional to assess patient needs and connect them with community agencies and resources based on referrals from clinicians.

The role requires 1-3 years of experience and a Bachelor’s degree in Human Services or a related field, plus a valid Maryland driver’s license and fleet clearance. Compensation ranges from $20.06 to $30.09 per hour depending on experience.

Qualifications

• 1-3 years of experience in related field.
• Bachelor's Degree in Human Services or related field required.
• Valid MD Driver's License required and clearance by the LBH Fleet Safety Department to operate vehicles.

Responsibilities

• Conducts an initial assessment and gathers data relevant to patient needs.
• Assesses patient needs and capabilities in the context of specific referral requests made by the primary clinician.
• Gathers and documents all necessary information needed for making patient referral, e.g. proof of patient income, current medication dosages, etc.
• Coordinates services and resources to meet patients ancillary needs.
