Job summary

Community Counseling Center in Cape Girardeau, MO is seeking a Community Support Caseworker to deliver community-based services for adults and children enrolled in the CPRC. The role involves working in client homes or residential centers, maintaining a caseload of 20–25 clients and meeting with them weekly.

Responsibilities include providing support services, training in daily living skills, coordinating with the treatment team, and referring clients to community resources.

Qualifications

• Bachelor's Degree, or four-year combination of higher education and qualifying experience, or four years of qualifying experience.
• Four years of qualifying experience.
• Qualifying experience includes service delivery to individuals with mental illness, substance abuse disorders, or developmental disabilities.

Responsibilities

• Provide community support services to children and adults enrolled in the CPRC.
• Maintain a caseload, including community and hospital settings.
• Provide training/education to clients in daily living skills.
• Monitor treatment plan goals and maintain contact with the treatment team.
• Refer clients to appropriate community resources to improve functionality and develop a support system.
• Document client interactions per agency guidelines, completing all documentation by deadlines and according to clinical policies, DMH, CARF, and federal/state program standards.
