Job summary

Health Connect America seeks a full-time case management professional to provide quality services for children, families, and adults, ensuring documentation and regulatory compliance. The role supports connecting clients with community resources and coordinating with treatment teams across a multi-state footprint.

The ideal candidate holds a Bachelor’s degree in human services and is provisionally credentialed as a PCSS in Mississippi, with eligibility for CSS.

Qualifications

  • Bachelor’s degree in human services discipline, with experience in case management or community resources.
  • Provisionally credentialed as a PCSS in Mississippi, with eligibility to become CSS.

Responsibilities

  • Provide best-practice, quality case management services in line with payer, state, and COA standards.
  • Assist in marketing referrals and communicating with customers and colleagues to answer questions.
  • Make required contacts when wraparound facilitator is unavailable, and document accordingly.
  • Connect families with needed community resources and follow up on referrals.
  • Complete documentation within established time frames.
  • Attend weekly Treatment Team meetings and participate with the treatment staff.
  • Serve as a liaison with other professionals and agencies for the client.
  • Develop a recovery support plan within the first 30 days of referral.
  • Provide 24/7 on-call support as per program requirements.
  • Ensure compliance with all state regulatory bodies.

Responsibilities

Provide best practice, quality case management services, ensure that all documentation is completed per Payer, State, and COA standards and requirements. Assist in marketing new referrals.

  • Communicate with customers, colleagues, and other individuals to answer questions, disseminate or explain information, and address complaints/concerns.
  • If no wraparound facilitator is present with MYPAC, CSS will make 2 contacts per week by phone or face to face and document accordingly.
  • Connect families with needed and available community resources. Follow-upwith clients and agencies as appropriate to document use/success of referral.
  • Complete documentation within established time frames.
  • Meet with clients weekly or bi-weekly for up to 12 months. (see above for MYPAC requirements).
  • Attend and participate -weekly with Treatment Team meetings.
  • Serve as a liaison with other professionals and agencies related to the client.
  • Complete a recovery support plan with all individuals within the first 30 days of referral to services.
  • Provide 24/7 on-call support for clients as per program requirements.
  • Ensure compliance with all state regulatory bodies.
Qualifications
  • Bachelor’s degree in human services discipline, including, but not limited to: Social Work, Psychology, Sociology, Counseling, Criminal Justice, etc.) and experience working with children and families in case management type/ community resource position.
  • Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CSS in Mississippi.
