Job summary

Health Connect America in Tennessee is seeking a Family Support Specialist to provide best-practice clinical services and assist families in accessing needed resources across home, community, and office settings. You will collaborate with the treatment team to develop plans, deliver individual, group, and family interventions, and ensure timely documentation.

The role includes weekly team meetings, referrals, and 24/7 on-call duties where required.

Qualifications

• Bachelor's degree in a human services discipline such as Social Work, Psychology, Sociology, Counseling, or Criminal Justice, with 1 year of experience with children preferred, OR a BA/BS with 3 years of relevant experience.

Responsibilities

• Communicate with customers, colleagues, and other individuals to answer questions, disseminate information, or address complaints.
• Develop treatment plans with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building as outlined in the treatment plan.
• Implement Health Connect America approved therapeutic interventions with clients and families.
• Complete all client documentation within established timeframes.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with services and resources identified in the treatment plan.
• Serve as a liaison with other professionals and agencies.
• Recruit new referrals to maintain caseload.
• Provide 24/7 on-call support as per program requirements.
• Ensure compliance with state regulations and COA standards.
• Participate in training and professional development, including E-learning.

The Family Support Specialist provides best practice, quality clinical services, ensures all documentation is completed per Payer’s, state licensing agent’s, COA, & HCA’s standards and established guidelines and assist with marketing for new referrals.

• Provide 24/7 on-call support for clients as per program requirements
• Ensure compliance with all state regulatory bodies & COA standards.
• Participate in and complete all assigned training and professional development activities, including E Learning courses.
• A Bachelor's degree in a human services discipline such as Social Work, Psychology, Sociology, Counseling, or Criminal Justice, along with 1 year of experience working with children (including internships or volunteer experiences) is preferred, OR a BA/BS degree with 3 years of relevant experience.
