Responsibilities include documenting services accurately, re-engaging clients after missed appointments, and mentoring colleagues. Bachelor's in Human Services is required; experience is not necessary.

Formación

• Bachelor's Degree in Human Services or related field.
• No prior work experience required.
• Licenses & Certifications listed: Class D (IL) or Class E (MO).

Responsabilidades

• Provides community support services to complex clients to include clients on care plan, using treatment plan interventions that result in positive outcomes.
• Completes timely documentation of services with details of intervention, dates, settings and client response.
• Contacting individuals/referral sources after missed appointments to re-engage and promote recovery.
• Provides mentorship and job shadowing to community support colleagues.
