Job summary

BJC Behavioral Health is seeking a Community Support Specialist in Farmington, MO to provide community-based services, including home visits, based on individual care plans. This role requires travel with personal vehicle and face-to-face client meetings to support recovery from mental illness.

Preferred candidates have a Master’s degree in Human Services and 2–5 years of relevant experience; training will occur during the first 90 days, with strong tech usage and documentation skills emphasized.

Qualifications

• Master's Degree in Human Services or related field.
• 2-5 years of experience in community support or related field.
• Valid Class E (MO) or Class D (IL) driver's license.
• Ability to conduct home visits and travel as required.
• Strong documentation and communication skills.

Responsibilities

• Provides community support services to complex clients according to treatment plans.
• Completes timely documentation of services describing the need, intervention, and client response.
• Contacts individuals or referral sources after missed appointments to re-engage client recovery.
• Offers mentorship and job shadowing to community support colleagues; supports clinical teams.
• Completes annual assessments and other required client documentation.
