Job summary

Center for Human Development (CHD) seeks a Community Support Program Worker in Springfield, MA. The role provides outreach and support services to clients with psychiatric or substance use disorder diagnoses to help access outpatient treatment and related supports.

You will coordinate services, link clients to benefits and housing, and participate in crisis prevention planning while leveraging natural supports and peer services to aid recovery.

Qualifications

• Bachelor’s degree or at least three years of relevant work or lived experience.
• Ability to pass certification for CPR, First Aid.
• Valid driver’s license; reliable transportation for business use.

Responsibilities

• Provide service coordination and linkages to the community.
• Provides outreach services to clients, delivering mobile services in community or institutional settings including inpatient units, homes, day programs, etc.
• Assist clients in obtaining benefits, housing and health care as needed
• Provide crisis support and participate in the development, revision and utilization of crisis prevention and/or safety plans.
• Encourages and facilitates utilization of clients’ natural support systems as well as recovery-oriented, peer support and self-help support and services.

The Community Support Program Worker provides outreach and support services to clients with psychiatric, or substance use disorder diagnoses to enable them to utilize clinical treatment services and other supports. The CSP Worker assists clients to attain his/her clinical treatment plan goals in outpatient and other levels of care and works to mitigate barriers to success.

• Provide service coordination and linkages to the community.

• Assist clients in obtaining benefits, housing and health care as needed

Requirements

• Bachelor’s degree from an accredited educational institution or at least three years of relevant work or lived
• experience that is acceptable by MBHP waiver.
• Ability to pass certification for CPR, First Aid.
• Valid driver’s license; reliable transportation for business use.

Pay Rate: $20/hr.

Location: Springfield, on-site

Schedule: Full-time, Monday through Friday, 9am-5pm
