Job summary

The Center for Human Development (CHD) is seeking a Community Support Program Worker for our Springfield outpatient clinic. You will provide outreach and linkages to the community, coordinate services, and support clients to achieve their treatment goals across outpatient and other care settings.

Responsibilities include delivering mobile outreach, aiding with benefits, housing and health care access, and participating in crisis planning and safety measures.

Qualifications

• Bachelor’s degree or at least three years of relevant work or lived experience.
• Ability to pass certification for CPR, First Aid.
• Valid driver’s license and reliable transportation for business use.

Description

The Community Support Program Worker provides outreach and support services to clients with psychiatric, or substance use disorder diagnoses to enable them to utilize clinical treatment services and other supports. The CSP Worker assists clients to attain his/her clinical treatment plan goals in outpatient and other levels of care and works to mitigate barriers to success.

• Provide service coordination and linkages to the community.
• Provides outreach services to clients, delivering mobile services in a number of community or institutional settings including inpatient units, homes, day programs, etc.
• Assist clients in obtaining benefits, housing and health care as needed
• Provide crisis support and participate in the development, revision and utilization of crisis prevention and/or safety plans.
• Encourages and facilitates utilization of clients’ natural support systems as well as recovery-oriented, peer support and self-help support and services.

Requirements:

• Bachelor’s degree from an accredited educational institution or at least three years of relevant work or lived
• experience that is acceptable by MBHP waiver.
• Ability to pass certification for CPR, First Aid.
• Valid driver’s license; reliable transportation for business use.

Pay Rate: $20/hr.

Location: Springfield, on‑site

Schedule: Full-time, Monday through Friday, 9am-5pm
