High Point is seeking a Community Support Navigator to deliver CSP/CSP-JI services in Hyannis, MA. The role requires a Bachelor’s in a behavioral health field and at least two years of relevant experience, plus a valid driver’s license and ability to work with diverse populations.

Qualifications

• Bachelor’s degree in a related behavioral health field and 2 years of relevant work experience.
• Valid driver’s license and ownership of a car.

Responsibilities

• Provide CSP and CSP-JI services as indicated in needs assessment/service plans.
• Coordinate with Mobile Crisis Intervention and outside providers.
• Maintain documentation of client progress and required billing/records.
