Job summary

Qualifications

• High School graduate or equivalent.
• At least two years of experience working with persons with disabilities or in a mentor/teaching position.

Responsibilities

• Support individuals to live independently in their community.
• Mentor and build relationships to connect individuals to community organizations and activities.
• Assist with activities of daily living as needed (cleaning, cooking, laundry, medication support, bill paying, personal care).
• Develop community relationships that foster connections between the person supported and community organizations.
• Make decisions and work independently.

OTHER EXPERIENCE AND
QUALIFICATIONS: Qualifications: High School graduate or equivalent At
least two (2) years of experience working with persons with disabilities
or in a mentor/teaching position that assists individuals in learning
Skills And Providing Them Independently. Experience And Understanding Of
community resources and methods of teaching independent living skills
Valid driver\'s license with good driving record and insured vehicle
