About the job Community Integration Specialist

Position Summary:

This position is responsible for providing direct support, teaching,and facilitation to enable participants with disabilities to engage inchosen activities are in line with their person-centered plan.

Duties and Responsibilites:

  • Provides individual and group supervision and instruction to clients both in the community and within the program facility.
  • Provides input into the development of Individual Service Plans.
  • Provides follow through on clients ISP goals/objectives to assurethat they are properly implemented, monitored, documented and evaluatedto meet program requirements.
  • Records daily progress made by client in meeting ISP goals and objectives.
  • Sets up instructional/training supplies each day as directed by the Program Manager or his/her designee.
  • Attends regularly scheduled staff meetings with Program Manager and other related personnel.
  • Assists in developing and implementing non-aversive behavioral management plans to meet the needs of the client.
  • Operates personal or agency-owned vehicle in a safe manner and in accordance with California State law.
  • Assists with cleanup of program facility and ensure that all materials are put away in appropriate location.
  • Monitors and assists clients arriving and departing the programincluding program vehicles and outside transportation departments.
  • Assists in personal care needs of clients including but not limitedto toileting, dressing, feeding, personal hygiene, lifting andpositioning.
  • Ensures clients safety with regard to special adapted equipment, i.e., wheelchairs, trays, speech devices.
  • Maintains a professional demeanor with management, co-workers, members of the community, and with all outside agencies.
  • Maintains Code of Safe Practices for his or her classification.
  • Dispenses medication to clients following Program Managers directives.
  • Adheres to the guidelines found in the Employee Handbook.
  • Adheres to the Rights of Persons with Developmental Disabilities,maintain client confidentiality, follow required reporting procedureswith respect to special incidents or suspected abuse.
  • Advocates for client rights and needs.
  • Completes and submits in a timely fashion, documentation of eachclients progress including daily session notes, monthly progressreports and daily activity sheets.
  • Assists clients with unanticipated or emergency situations that may arise.
  • Selects or develops resource materials to enable client growth.
  • Participates in in-service training as required and maintains required certification.
  • Adheres to guidelines for conducting Community Based Instruction.
  • Other duties as assigned.

Required Skills/Abilities:

  • Excellent verbal and written communication skills.
  • Ability to create and implement a tracking system for all clients served.
  • Proactive and independent with the ability to take initiative.
  • Excellent time management skills with a proven ability to meet deadlines and monthly/annual goals.
  • Has the ability to quickly learn new systems as they are implemented.
  • Proficient with Microsoft Office Suite, videoconferencing using Zoom, Webex, etc., and adequate/reliable internet access.

Education and Experience:

  • CPR and First Aid certification (can be provided by UCP)
  • Valid driver license and clean driving record, Insurance.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer and/or talking on the phone.
  • Must be able to occasionally lift and/or carry up to 35 pounds.
