Job summary

The Arc of Opportunity in Fitchburg, MA is seeking a Community Support Coordinator to develop curricula and identify community resources aligned with ISP goals. This full-time role involves coordinating schedules, supporting participants, and ensuring service standards.

You will work with program directors to implement Individual Support Plans, document activities in the EMR, and facilitate transportation to community activities, with a Monday–Friday schedule from 8am to 4pm.

Qualifications

  • High school diploma or GED required.
  • Valid driver’s license and CORI on file.
  • Ability to assist with personal care needs.

Responsibilities

  • Deliver service standards and ensure regulatory compliance within the assigned program.
  • Implement Individual Support Plans based on assessments and goals.
  • Create daily/weekly employment and skill development schedules aligned with ISP goals.
  • Develop employment curriculum for participants.
  • Submit required electronic documentation, including monthly work site evaluations and billing.
  • Code services accurately in the EMR for billing purposes.
  • Maintain positive relationships with community work sites.
  • Facilitate group/programming and maintain a clean program space.
  • Provide transportation to participants for community activities.

Develops curricula and identifies community resources to build volunteer and preference-based opportunities for participants that links to the ISP goals.

Schedule: Monday-Friday 8am-4pm

Essential Functions:

  • Works closely with Person Centered Plan Managers and Program Directors in delivering service standards of excellence and ensuring operational compliance within assigned program in accordance with federal, state and agency regulations.
  • Implement Individual Support Plans for individuals based on assessments, objectives and goals.
  • Create daily and weekly employment and skill development schedules based upon participants ISP goals, skills, and preferences.
  • Develop employment curriculum.
  • Complete and submit required electronic documentation, which includes monthly work site evaluations, attendance, billing and production sheets.
  • Timely and accurate coding for services into electronic medical records system for billing purposes.
  • Under the guidance of PD of Volunteer and Career development maintain a positive, professional relationship with community work sites.
  • Maintain ratio and facilitate group/programming as needed.
  • Responsible for maintaining a clean, accessible program space in assigned home room area.
  • Ability to access all modes of transportation including driving and public transportation.
  • Attend mandatory trainings and meetings as required and assigned.
  • Actively advocates for participants.
  • Assists individuals with skill building and attainment of ISP/service plan goals.
  • Provides transportation to participants to community activities including performing safe lifts and transfers.
  • Demonstrates commitment to agency mission and The Way of The Arc in all interactions.
  • Other duties as assigned.

Required Qualifications:

High school diploma or GED is required.

  • A valid driver's license and acceptable driving record and CORI is required at all times during employment with The Arc of Opportunity.
  • Ability to transfer, change, shower assist with toileting and all personal care needs of the individuals supported.

Physical Requirements:

  • Ability to lift up to 50 lbs., kneel, bend and carry items, and sustain periods of walking, standing, and sitting.
  • Ability to physically assist individuals according to their needs and perform physical restraint if necessary.
  • Cross functional communication skills in written and oral
  • English which conveys clear and concise information in a professional and respectful manner to individuals supported, family members/guardians, peers, government agencies, community partners and employees.
  • Functional and situational reasoning to follow policy, problem solve, exercise sound judgment, prioritize multiple tasks, and manage crisis.
